Fill and Print out your individual voucher
1) Open the corresponding PDF-Form in free Adobe PDF Reader
If you bought a value voucher, please fill and print out the "Voucher in CHF" form.
If you bought a voucher for a specific treatment, please fill and print out the "Treatment Voucher" form.
2) Fill out all the open fields in the PDF-Form
It is essential that you indicate the
- transaction code (on the top right on the PayPal email)
- purchase date
- and either CHF value or treatment.
3) Print out the PDF-Form and make someone happy :)
